LIONS CLUB HEALTH SCIENCE SCHOLARSHIP
For Non-Traditional Students’ Continuing Education
APPLICATION
Scholarship awards will be based on the following criteria: Resident of
Frederick County, Maryland;
financial need; academic achievement and personal qualifications.
A letter of reference dated after the applicant receives the application, and a
copy of the most recent college
or post-high school transcript are to be mailed by the references directly to
the Scholarship Chairman.
The letter of reference is to be from: a current teacher or administrator (if
you are currently enrolled in school);
your supervisor (if you are not in school but are employed); or a non-relative
who has known you for at least
five (5) years.
Mail the completed application to:
Scholarship Committee, FSK Lions Club, P. O. Box 3932,
Frederick, MD 21705-3932.
Application and supporting documentation must be received by May 15, 2011.
Applications received after the deadline will not be considered.
